However, similar issue, see image attached. I do not know what "Ox..." address was used or even how to enter it.
But I went through this process with the beta wallet, clicked "add an extra word" entered my password and addresses with zero balances.
What am I missing?

Please ensure that you are entering the correct password, and also check for case sensitivity. Give it a try once more.
I'm unable to view the attached screenshot/image. Could you please share it again?

See if this works. I tried it again without the password and same thing.
